Soccer Uniform Information

To bring you the best quality uniforms at the best price with the best service we are now working with Sports Distributors in Richardson.

Head coaches or a team representative must contact Sports Distributors directly for all of their uniform ordering details.  Coaches will organize the uniform order by using the forms listed below.  Parents need to contact their coach to supply uniform size.

New Team Uniform Ordering Process for Coaches:

1.  Select the uniform of your choice by visiting the High Five website.  Please keep in mind that our prices are lower than the prices listed on this website.

2.  Gather sizes and number choices from your players.

3.  Click HERE for a Word Document of the order form. (Information can be typed in)

4.  Click HERE for a PDF of the order form.

5.  Submit form to Sports Distributors by fax (972-235-4123) or by email to donsportsd@aol.com

Returning Team Uniform Ordering Process for Coaches:

1.  If your brand is High Five and you need fill ins, click HERE to see the updated uniform choices.  If you are ordering a complete new set of uniforms, see the new team process above.

*Uniform orders must be submitted to Sports Distributors by the uniform order deadline (see key dates).
